Yes! We are now in-network with Blue Cross Blue Shield (BCBS), which fully covers the Initial Ketamine Evaluation, as well as follow up appointments for Medication Management, Psychotherapy, and Integration Sessions. (Deductibles, co-insurance, and copays still apply in accordance with your specific insurance plan.)
Ketamine administration during a ketamine-assisted therapy session is NOT a covered service and is billed at the self pay rate. If you have an HSA/FSA account, ketamine treatment is an eligible medical expense and we accept this form of payment.
For those with health insurance providers other than BCBS, we provide invoices called “superbills” for you to claim out of network benefits with your insurance. Amount of reimbursement varies by plan and is not guaranteed. Not all insurance plans offer out of network benefits.
If you plan on using your out-of-network benefits, we strongly recommend that you contact your insurance ahead of time to get an estimate of how much they will reimburse. Our billing codes are listed below:
Initial Ketamine Evaluation: 90792
Medication Management (30 minutes): 99214
Psychotherapy (Integration) without Medication Management (45 minutes): 90834
Psychotherapy (Integration) without Medication Management (60 minutes): 90837
When you contact your insurance please make sure they understand that you will be receiving services from an "out of network mid-level provider" as this will factor into their reimbursement rates.

Yes! As we offer 3-month and 6-month payment plans on all 6-session ketamine treatment bundles. Payment plans are available with no interest and at no additional cost to you.
We also accept all major credit cards, HSA/FSA, personal check, Venmo, and cash.

No. As a psychiatry practice, we are only able to provide treatment for primary psychiatric disorders. Although some clients with co-occurring chronic pain do receive some pain relief using sublingual ketamine, it is often the case that more robust pain relief requires higher doses than can be administered sublingually. For this reason, the most common route of administration for this indication is IV, which we do not offer at this time.

We do offer Medication Management to our ketamine patients who do not have a psychiatric prescriber and are looking to optimize their psychiatric medication regimen after completing their 6-session ketamine induction series.
Both our Nurse Practitioners are in network with Blue Cross Blue Shield only, and self-pay for all other insurances (superbills can be provided for you to claim out of network benefits with your insurance if applicable).
